Patient Portal Log-in

As a secure and confidential platform, the Patient Portal provides students the opportunity to:

  • Schedule and access appointments. (Please call 315.443.8000 to schedule a Counseling or Health Care appointment.)
  • Securely communicate with the health and wellness team.
  • Access and submit records.

Using the Patient Portal

Only Syracuse University students have access to the Patient Portal.  For questions or additional information, please email the Barnes Center at The Arch or call 315.443.8000.

Communications, Scheduling and Requests

Students are encouraged to enroll in to receive health and wellness text message notifications from the Barnes Center at The Arch. Highlights include appointment reminders, secure message notifications, important health and wellness updates, and more.

  1. Log into the Patient Portal with your Syracuse University email and password.
  2. Select “Forms,” then under “Student Forms,” select “Communications Preference.”
  3. Please follow the directions on the form to complete enrollment.
  4. At any time you may text *HELP* for additional assistance or *STOP* to cancel.

  • Emergency: If you are experiencing a life-threatening emergency or you are in immediate danger, call 911.
  • Non-emergency Well Visits: All non-emergency, well visits and immunization appointments may be made with the Patient Portal.
  • More Urgent Non-emergency Appointments: More urgent non-emergency appointments, such as sick visits may be scheduled starting after midnight the night before the desired visit date.
  • Multiple Health Concerns: If you are having multiple health concerns, please select multiple appointment visit types.
  • Same Day/Sick Appointments: Please note, sick appointments can only be made as “Same Day” appointments. The “Same Day” appointments open at midnight each day. If you are trying to make a sick appointment for later in the week, you will either need to call Health Care at 315.443.8000 or schedule a “Same Day” appointment on the day you would like to come in.
  • Recurring Health Concerns: If you were seen recently for the same concern, contact the Barnes Center at The Arch to speak with your health care provider's nurse by calling 315.443.8000.

What if I did not find an appointment visit type that closely meets my needs?

Some appointments require prior discussion with a health care representative before booking. If you do not find an appointment visit type that closely meets your needs, email the Barnes Center at The Arch or call 315.443.8000 to schedule an appointment.

Failure to show for a scheduled appointment or failure to call to cancel at least two hours prior to an appointment, will be assessed a "no show" fee on the student’s account.

Scheduling an Appointment (Counseling, Health Care, Nutrition) in the Patient Portal

  1. Log into the Patient Portal with your Syracuse University email and password.
  2. Select “Appointment” on the top bar.
  3. Under “Welcome to Self Scheduling,” read the information before moving forward.
  4. Under “Select a Reason for Your Appointment,” select the symptom(s) you would like to be seen for and complete each drop-down menu. If you cannot find it on the list or have multiple symptoms that are not related, please call Health Care at 315.443.8000.
  5. Under “Provider” select a specific provider or select “all” for any provider. If scheduling for a prescription refill, you will need to choose the provider who has prescribed it to you.
  6. Select “Submit” when complete and read the pop-up instructions before continuing.
  7. Next select the date and time for your appointment.
  8. Please bring a hardcopy of your insurance card and arrive five minutes prior to your appointment to complete check-in paperwork.

Mobile check-in is required prior to entering the Health Care waiting room.

  1. Log into the Patient Portal with your Syracuse University email and password.
  2. Check-in on the Patient Portal.
  3. Complete all forms.
  4. Verify insurance at the front desk.

Refilling a Prescription

  1. Log into the Patient Portal with your Syracuse University email and password.
  2. Select Forms.
  3. Then complete the form under “Pharmacy.”

Requesting a Prescription Transfer

  1. Log into the Patient Portal with your Syracuse University email and password.
  2.  Select Forms.
  3.  Scroll down to Pharmacy and fill out the form for “SUHS Pharmacy Refill/Transfer Request Form.”

Using the Patient Portal, students may receive messages from health care providers containing next step instructions. To access messages from health care providers, log into the Patient Portal with your Syracuse University email and password.

IMPORTANT: Please bring your pharmacy benefit card and health insurance card with you to your appointment. The flu vaccine will be billed to your insurance. Most health insurance companies cover the cost of a flu vaccine in full. Students who have a copay or medical charge applied will be billed accordingly. Please know ability to pay will not impact your access to services. If you are actively enrolled in the Syracuse University Student Health Insurance Plan, you do not need to bring your insurance information.

  • Log into the Patient Portal with your Syracuse University email and password. (You will be prompted to complete Microsoft’s Multi-factor Authentication in advance of accessing the Patient Portal if you have not done so previously.)
  • Select “Appts” on the main menu.
  • In the drop-down section:
    • Under “Clinic,” select “Health Clinic.”
    • Under “Reason,” select “Flu Shot Clinic Appointment.”
    • Under “Provider,” select “Barnes Center 309 FluShots.
    • Select “Submit.”
  • Select your appointment day and time.
  • After signing up, you will receive a confirmation to your Syracuse University email with appointment details and needed forms.

In partnership, Student Association (SA) and the Barnes Center at The Arch, invite Syracuse University students to participate in free Sexually Transmitted Infections (STIs) screening clinic appointments throughout the academic year. Participants of the Student Health Insurance Plan receive free STIs testing at any time.

Confidential STIs clinics provide screening opportunities for the following.

  • Chlamydia: Rectal, Throat, Urine
  • Gonorrhea: Rectal, Throat, Urine
  • Human Immunodeficiency Virus (HIV): Blood Test
  • Syphilis: Blood Test

Please note, appointments are first come, first serve and scheduling will only be open during the following appointment times.

  1. Log into the Patient Portal with your Syracuse University email and password.
  2. Within the top banner select “Appts.”
  3. Under “Search” select “GYT Event STI Screening Clinic.”
  4. When prompted, enter "GYT" as the appointment code.
  5. Select “Submit” to complete.

Clinic appointments are available between 9 a.m. and 3:30 p.m. on the following dates.

  • Feb. 10-11, 2022
  • Feb. 17-18, 2022
  • March 31-April 1, 2022
  • April 7-8, 2022

As of Monday, Sept. 27, 2021, the Patient Portal added Microsoft’s Multi-factor Authentication (MFA) to the login process. This is the same multi-factor authentication you already use to access SUMail, and that you use to access MySlice as of Sunday, Sept. 26, 2021. For additional details regarding configuring and using MFA, please refer to Microsoft Two-Factor Authentication in Answers.

Submitting Forms and Waivers

  • Log into the Patient Portal with your Syracuse University email and password.
  • Select the “Immunizations” tab from the top menu, and select “Enter Dates”
  • Under the “Recommended Immunizations” heading, find the type of COVID-19 vaccine you received, enter the dates(s) and click Submit.
  • Once the date(s) have been submitted, select the “Upload” tab from the top menu.
  • Follow all upload instructions.
  • In the drop-down menu labeled “Choose Document,” select “COVID Proof of Vaccination.”
  • Click “Select a file,” choose the file containing your vaccination documents, and click “Upload.”

There are two parts to submitting records—recording the date of your booster vaccine and uploading proof of your booster vaccine. Your record is not considered complete until a copy of your immunization proof has been uploaded for verification.

  • Log into the Patient Portal with your Syracuse University email and password.
  • Select the “Immunizations” heading and then “Enter Dates.”
  • Find the type of COVID-19 booster vaccine you received, enter the date, and select “Submit.”
    • IMPORTANT: You do not need to re-enter your initial vaccine series information. Please only enter your booster as Dose #3 for the vaccine type that you received, even if different from your initial COVID vaccination series.
  • Once the date has been submitted, select the “Upload” tab from the top menu.
  • Follow all upload instructions.
  • In the drop-down menu labeled “Choose Document,” select “COVID Proof of Vaccination.”
  • Click “Select a file,” choose the file containing your booster vaccination document and select “Upload.”

In accordance with New York State law, the following vaccination records must be submitted. If you choose not to obtain the Meningococcal meningitis  immunization, please complete the Meningitis Waiver Form in the Patient Portal. Please note, if you are under the age of 18, a parent or guardian must complete the Minor Consent Form.

  • Two Measles, Mumps and Rubella (MMR) vaccinations.
  • Meningococcal meningitis vaccination within the last five years and after the age of 16.

You may obtain these records by contacting your high school or primary care physician. Your health care provider may complete this form if they do not have a copy of your immunization records. If you have any questions about your vaccination records, please contact your primary care physician. By appointment only, Barnes Center at The Arch Health Care is available to assist you in obtaining the appropriate vaccinations if you have not been vaccinated or do not have documentation of your vaccinations.

Patient Portal Upload Instructions

  • Select “Forms” on the top menu.
  • Required forms will be marked with a red asterisks (*).
  • Begin the process by selecting the form and then following the prompts to complete.

All students are required to complete the TB Screening. You (and for those under age 18, your parent or guardian) must complete in full. Please read each question carefully.

  • Log into the Patient Portal with your Syracuse University email and password.
  • Select “Forms” on the top menu.
  • Required forms will be marked with a red asterisks (*).
  • Begin the process by selecting the form and then following the prompts to complete.

  • Log into the Patient Portal with your Syracuse University email and password. (You will be prompted to complete Microsoft’s Multi-factor Authentication in advance of accessing the Patient Portal if you have not done so previously.)
  • Select “Forms” from the main menu.
  • Under Student Forms, select the “Medical-Religious Waiver” form to print the waiver and complete, including:
    • a doctor note sharing supporting details or,
    • a personal student written and signed statement if 18 years of age or older (if under, written by parent/guardian) outlining the reason the vaccine(s) conflict with sincerely held religious beliefs.
  • Once all are complete (e.g. waiver, doctor note, personal statement), select “Upload” on the home screen.
  • Follow the upload instructions.
  • In the drop-down section, select “Medical or Religious Waiver Form.”
  • Select the completed waiver file to upload.

  • Log into the Patient Portal with your Syracuse University email and password.
  • Select “Forms” on the top menu.
  • Select "Waiver Form-Meningitis"  and follow instructions to complete.

This form must be completed by a parent or guardian if you will not be 18 years of age by the time you begin classes. Please print this form and complete it in full. Once you have completed the form, you will need to upload it to the Patient Portal.

  • Log into the Patient Portal with your Syracuse University email and password.
  • Select “Forms” on the top menu.
  • Required forms will be marked with a red asterisks (*).
  • Begin the process by selecting the form and then following the prompts to complete.

  • Log in to the Patient Portal with your Syracuse University email and password.
  • Select the “COVID-19” heading.
  • Scroll down to the “Off Campus COVID-19 Lab Results” section.
  • Under “Enter External COVID-19 Lab Result,” enter the test date and result of test. (Note: only positive test results should be submitted.)
  • Select “Submit.”
  • The result you entered will appear on the screen.
  • Click “Select File” next to your submitted result to add an attachment showing your results. (Your full name and date of test must be clearly identifiable.)
    • If you received a formal result, upload a copy of the documentation.
    • If you did an at-home test, upload a photo of your test showing the positive result, with your Syracuse University I.D. visible next to the test.
  • Select “Upload.”
  • Please note it may take up to 48 hours for the results to be reviewed.

  • Log into the Patient Portal with your Syracuse University email and password.
  • Select “Upload” on the main menu.
  • Follow the upload instructions.
  • Select “Flu Vaccine Documentation” from the drop-down.
  • Add your proof of vaccination.

Check all medical conditions that you have had in the past or currently have. Students must fill out this part in full, for minors this is required by their parent or guardian. This form will provide information needed to best treat you, should you visit. Please remember that your health information is strictly confidential and cannot be shared with any other part of the University.

  • Log into the Patient Portal with your Syracuse University email and password.
  • Select “Forms” on the top menu.
  • Required forms will be marked with a red asterisks (*).
  • Begin the process by selecting the form and then following the prompts to complete.

This form gives the Barnes Center at The Arch staff permission to treat you at our clinics along with providing us an emergency contact. To sign, use a touch screen or the mouse on your computer.

  • Log into the Patient Portal with your Syracuse University email and password.
  • Select “Forms” on the top menu.
  • Required forms will be marked with a red asterisks (*).
  • Begin the process by selecting the form and then following the prompts to complete.

Accessing Records and Checking Status

  • Log into the Patient Portal with your Syracuse University email and password.
  • From the main menu select "Immunizations" and then "View History."
  • The “Overall Status:” header will display the current state of compliance.

Overall Status Definitions

    • Verified: No action needed and currently compliant with all requirements.
    • Compliant: Entered dates suggest compliance with requirements but documents are not yet verified. Document review may require three to five business days to complete. If further action is required, an email (@syr.edu) will be sent including additional needed actions.
    • Not Compliant: Pending or incorrect information. An email (@syr.edu) will be sent including additional needed actions.
    • No Data: No information or not enough information to determine compliance.

Check Missing Requirements

  • Log into the Patient Portal with your Syracuse University email and password.
  • From the main menu select "Immunizations" and then "View History."
  • Select the "Print" button.
  • The PDF report will display the following.
    • "Copies of Records Received"
      • "Verified" Column "Yes:" The Barnes Center has verified record.
      • "Verified" Column "No**:" The Barnes Center has not verified record and it is currently considered "Self-Reported." (Most often represents a record not yet reviewed. Students will receive an email (@syr.edu) if additional action is required. Do not re-upload unless asked to do so.)
    • Missing Requirements: Listed at the bottom.

  • Log into the Patient Portal with your Syracuse University email and password.
  • In the drop-down menu under “Immunizations,” select “View History.”
  • You may print Immunization Records by selecting “Print.”

Students may access charge details and/or retrieve an itemized receipt through the following actions.

  • Select the "Statements" tab.
  • Contacting the Barnes Center at The Arch Medical Records Office by calling 315.443.2667.
