Certificate of Liability Insurance

It is the responsibility of each chapter to make sure their insurance is kept up to date, as any lapse in coverage may result in an immediate halt in all chapter programming and activity on campus.

Chapter Roster Forms

All chapters are required to submit their rosters at the beginning and end of every Fall and Spring semester. Only rosters that are fully completed and submitted in the proper Greek Life roster template will be approved.

For the beginning of the Fall semester ONLY, chapters must submit their rosters with their re-registration paperwork.  Thereafter, chapters must submit their rosters using the Greek Life Roster Template [Excel] and submission forms:

Potential New Member (“PNM”) Recruitment/Intake Forms

• Before a student can go through recruitment/intake, they must submit a Potential New Member (“PNM”) Eligibility & Release Form.  First select council below and then chapter affiliation (if known) in the form itself:
