Certificate of Liability Insurance
It is the responsibility of each chapter to make sure their insurance is kept up to date, as any lapse in coverage may result in an immediate halt in all chapter programming and activity on campus.

Chapter Roster Forms

New Member Roster
All chapters must submit a New Member Roster when their New Member process is complete and members are initiated.
Eligibility & Release Form
Before participating in recruitment, membership, or intake activities, each student must submit an Eligibility & Release Form.
Before completing the form, select the appropriate Greek council you are interested in joining. If you’re unsure which council you are interested in joining, please visit the Councils & Chapters page to identify the organization before submitting the form.
Note: The Panhellenic Council has a separate process for determining recruitment eligibility. Students interested in Panhellenic recruitment should follow the eligibility requirements and registration process outlined on the Panhellenic Recruitment page.
