Certificate of Liability Insurance
It is the responsibility of each chapter to make sure their insurance is kept up to date, as any lapse in coverage may result in an immediate halt in all chapter programming and activity on campus.

Chapter Roster Forms

New Member Roster
All chapters must submit a New Member Roster when their New Member process is complete and members are initiated.
Eligibility and Release Form
Before participating in recruitment, membership, or intake activities, each student must submit an Eligibility and Release Form. Before completing the form, select the appropriate Greek council you are interested in joining. If you’re unsure which council you are interested in joining, please visit the Councils & Chapters page to identify the organization before submitting the form.
Note: The Panhellenic Council has a separate eligibility and registration process for Spring 2027 Primary Recruitment. Students interested in participating in Spring Primary Recruitment should follow the registration requirements and process outlined on the Syracuse University Panhellenic website. The COB Eligibility and Release process below only applies to Fall 2026 and does not serve as registration for Spring Primary Recruitment.
