Immunization Requirements

In accordance with New York Public Health Law Sections 2165 and 2167, all Syracuse University students who were born on or after Tuesday, January 1, 1957, and registered for six or more credit hours or residing in University housing, must demonstrate proof of immunization for measles, mumps, rubella and meningococcal disease prior to beginning classes. If applicable, only officially translated documents are accepted.

The Barnes Center at The Arch health and wellness team must maintain a record of the following for each student:

  • Documentation of two doses of measles, mumps and rubella (MMR) or proof of immunity.
  • Documentation of one dose of meningococcal meningitis ACWY vaccine within the preceding five years and after the age of 16, or a completed waiver form declining the vaccination.

  • You may acquire records by contacting your primary care physician, previous school or institution.
  • The Barnes Center Health Care team is available to assist students who have not been vaccinated or do not have vaccination records. Please call 315.443.8000 to learn more.
  • Visit the Patient Portal webpage for detailed instructions on accessing Immunization Records previously submitted to the Patient Portal.

  • Records may also be faxed to 315.443.9010.

  • Please visit the Patient Portal webpage for detailed instructions and upload documentation, including date, that the first dose of Measles, Mumps and Rubella (MMR) was received.
  • If a first MMR dose was less than 28 days ago, submitted documentation will be reviewed for compliance until eligible for the second MMR dose.
  • A second  MMR dose is not required until an individual is eligible. 
  • Once complete, visit the Patient Portal webpage for detailed instructions and upload documentation of second MMR dose as soon as possible to stay in compliance.

If there is concern with potentially missing a University immunization deadline, please email the Barnes Center Health Care team, including a student SUID and details as to why the deadline may be missed, as soon as possible. The Barnes Center team will then work to review and provide personalized guidance on next steps.

The following students are not required to submit proof of immunization.

  • Students born before Tuesday, January 1, 1957.
  • Students who do not reside in University housing and are registered for less than six credit hours.
  • Students exclusively matriculated in University online degree programs.

COVID-19 Vaccination and Booster Requirements

Syracuse University requires all students, faculty, and staff routinely accessing campus to be vaccinated against COVID-19. This includes receiving a booster shot. Individuals who are not yet eligible to receive a booster by check-in and move-in, will be required to receive a booster shot during the academic semester, within one week of becoming eligible. The only exception to these requirements is an approved medical or religious exemption filed with the University. Learn more about the COVID-19 vaccine policy

Accepted COVID-19 Vaccines and Boosters

Receiving a COVID-19 Vaccine or Booster that is Not Accepted

  • Individuals will be required to begin a series of one of the accepted vaccines and follow instructions for providing proof on the Patient Portal.
  • Individuals should wait at least 28 days after their last dose of a non-accepted vaccine to begin a new series.
  • If this results in potentially missing a University deadline, please email the Barnes Center Health Care team, including a student SUID, as soon as possible.

Provide Proof of COVID-19 Booster

Who is Eligible for the COVID-19 Booster Vaccine

  • Individuals who received their second dose of the Pfizer or Moderna vaccine at least five months ago.
  • Individuals who received the Janssen/Johnson & Johnson vaccine at least two months ago.
  • Individuals who previously received World Health Organization-approved vaccinations other than Moderna, Pfizer or Janssen/Johnson & Johnson should receive a Pfizer booster dose if they received their initial vaccine series at least five months ago, in accordance with Centers for Disease Control and Prevention guidance.

  • Please visit the Patient Portal webpage for detailed instructions and upload documentation available so far of accepted COVID-19 vaccine(s) and/or booster date(s).
  • If a first dose of an accepted COVID-19 vaccine was less than a month ago (21-28 days pending  accepted COVID-19 vaccine) and/or the second dose was less than five months ago, submitted documentation will be reviewed for compliance until eligible for the next vaccine dose and/or booster.
  • A second  accepted COVID-19 vaccine dose is not required until an individual is eligible. 
  • An accepted COVID-19 booster is not required until five months after an accepted COVID-19 vaccine series is complete.
  • Once complete, visit the Patient Portal webpage for detailed instructions and upload documentation of remaining accepted COVID-19 vaccine(s) and/or booster date(s) as soon as possible to stay in compliance.

If there is concern with potentially missing a University COVID-19 vaccine and/or booster deadline, please email the Barnes Center Health Care team, including a student SUID and details as to why the deadline may be missed, as soon as possible. The Barnes Center team will then work to review and provide personalized guidance on next steps.

As a reminder, the only exemption from this requirement is an approved medical or religious waiver on file with the Barnes Center. If you already have a medical or religious waiver on file with the Barnes Center for the COVID-19 vaccination, you do not need to take any further action. If you need to submit a medical or religious waiver, please complete and upload the "Medical-Religious Waiver Form" in the Patient Portal.


Influenza Vaccination Requirements 

Syracuse University requires all students to receive the influenza vaccine (“flu shot”) or, alternatively, should have filed a medical or religious exemption with the University. Learn more on the Stay Safe Pledge webpage.

PLEASE NOTE: Students must have an influenza vaccine or a documented exemption to attend in-person classes, participate in on-campus activities and use on-campus facilities.
